During your stay you and your doctor will make decisions about your care. Blue Mountain Health System will inquire of all adult patients admitted to the hospital whether they have written advance directives, such as a living will, durable power of attorney for health care, psychiatric advance directive, or appointment of a health care proxy. This document gives your physician guidance about your medical treatment if you are unable to convey your wishes and is included on your chart at the time of your admission.
